The purpose of this form is to create a direct channel for DAT3 Staff members to assist the OTAD Driver on the road with minimal disruptions. Once the Driver complete the form, an email will automatically be sent to two DAT3 Staff members. The Staff member will complete the request and the driver will have access package that was previously unavailable/undeliverable.
